You agree to participate in live, online group exercise classes, recorded group exercise classes and informational seminars with Diane Markovitz PT, DianePT.com and MDL Wellness, LLC.
You understand and agree that it is your sole responsibility to consult with your physician regarding your physical condition if you have any concerns about whether you may safely participate in these live or recorded exercise classes and informational seminars generally or in any activity you may undertake during a session.
You, on behalf of yourself, your personal representatives and heirs do hereby release Diane Markovitz PT, DianePT.com and MDL Wellness, LLC from any and all claims, actions, or losses for bodily injury, property damage, wrongful death, loss of services or otherwise which may arise due to your performance of any activity during any live group exercise class, recorded group class or informational session.
You are fully aware that you are participating in these sessions at your own risk, and you will not hold Diane Markovitz PT, DianePT.com and/or MDL Wellness, LLC responsible in the event you incur an injury or exacerbate any physical condition existing prior to or arising during participation. You understand that by agreeing to these terms and conditions, you release Diane Markovitz PT, DianePT.com and MDL Wellness, LLC of any and all liability for your health and safety during your participation in these live exercise classes, recorded exercise classes and informational seminars.
